• Are you aged 40 or older?
• Do you have a parent, brother, sister or child with colorectal cancer?
• Do you have a parent, brother, sister, or child who has had colon polyps?
• Do you have inflammatory bowel disease known as ulcerative colitis or Crohn's disease?
• Have you experienced any of the following symptoms?
• Blood in or on the stool or passage of bright red blood
• Change in bowel habits such as diarrhea, constipation or having stools narrower than usual
• Unexplained and frequent aches, gas pains, or cramps in the stomach
• A feeling that you need to have a bowel movement that is not relieved by doing so
• Weight loss or loss of appetite for no apparent reason
• Weakness and fatigue
